We are working to automate these procedures, but for now print out the desired form on white paper and fill out the information in as much detail as possible. Please bring in the forms at your next office to help reduce your wait time.

Please see the enclosed list of currently accepted insurances and links to their sites. We will update this list as necessary. If you do not see your insurance listed, please feel free to give us a call at (850) 219-2273 to see if we accept your insurance.

Billing Information

As of January 2024, the patient portal is no longer active. We hope to have an update on a new portal soon.

Statements from Wilson Family Medicine
To pay for the statement you have received for Wilson Family Medicine, you can pay online (through Stripe) or call us. If you have a question regarding the statement you have received you can submit a request online or call us. As always you are more than welcome to bring your statement in and pay at the office.

Statements for Laboratory Services
If you have received a bill from the laboratory for your lab services and believe they were billed in error, please submit a request online (don’t forget to attach your invoice from Quest, Labcorp, etc.) or bring in your lab bill and we will see if it can be rebilled. We cannot guarantee your lab services will be covered as the doctors are ordering the labs as they believe they are necessary for your condition.
